The behind the scenes section said page 711 of Harry Potter and the Order of the Phoenix was the scene when Sirius died. I just checked and 711 is nowhere near that part. Page 711 has Umbridge, and I don't remember Umbridge being in the Department of Mysteries. Sirius' death was on page 806. That is way off. I'm holding the book as I'm typing this. - According to this, the reference appears on page 315 of the British version of The Prisoner of Azkaban. However, I live in the U.S., so I can't check this right now, nor do I have a copy of the U.S. edition on me to check if it appears in that version.
