Of Sybill Trelawney's two true prophecies, the second was the first one of which Harry was aware. The prophecy goes as follows:

"It will happen tonight. The Dark Lord lies alone and friendless, abandoned by his followers. His servant has been chained these tweleve years. Tonight, before midnight... the servant will break free and set out to rejoin his master. The Dark Lord will rise again with his servant's aid, greater and more terrible than ever he was. Tonight... before midnight... the servant... will set out... to rejoin... his master..."

The prophecy is made by Sybill Trelawney to Harry Potter. Harry believed she was referring to Sirius Black as the servant, but she meant Peter Pettigrew. The prophecy was made during Harry's third year final exam in Divination.
