It is assumed that her house-elf Hokey did it, her old age causing her to mistake the poison for sugar when making her mistress's evening cocoa. However, it was actually Tom Riddle's doing, so he could steal Salazar Slytherin's Locket and Helga Hufflepuff's Cup, which were in Hepzibah's possession, while implanting a false memory into the elf to frame her.
